A Clear Path for Life After Mom & Dad™

The Future Is Now™

If it feels like you were dropped into the deep end, you’re not alone.
Many siblings step into responsibility feeling unprepared, not because they don’t care about their sibling, but because the future was never fully talked through.

The Future Is Now™ gives you clear direction, personalized guidance, and momentum that builds over the next 3–6 months, so your sibling gets the support they need and you can both live fuller lives.

You Shouldn’t Have to Do this Alone

Disability care services are complex. The decisions are heavy. And trying to piece everything together without support is exhausting.

That’s exactly why The Future Is Now™ exists. We give you clarity, guidance, and a steady partner who knows how to move things forward.

What You’ll Walk Away With

By the end of the program, you will have:

Clarity on what your sibling truly needs: Not a medical or caregiving plan, but a clear understanding of safety priorities, service gaps, and the supports that are realistically required to keep your sibling stable and supported.

A realistic path through the system: You’ll know which services are worth pursuing, which aren’t, and what steps will actually move your sibling toward real support, without wasted time or false hope.

Clear expectations of what the system will (and won’t) provide: So you’re no longer guessing what Medicaid, agencies, and programs are responsible for and what still sits with the family.

Housing direction informed by service eligibility:  Not a mandate about where your sibling “should” live, but clarity on how living arrangements impact safety, access to services, and long-term stability.

A prioritized roadmap for what comes next: You’ll leave knowing what needs to happen first, who needs to be involved, and which actions will unlock forward
progress.

Hands-on support while services are being opened: You’re not sent off alone with a checklist. You’ll have guidance and follow-through as applications, approvals, and system steps move forward.

Relief because of clarity and direction: Not because everything is finished, but because you finally know what to do next and how to move forward with confidence.

How It Works

A guided, human-first process built for overwhelmed siblings.

Step One

We get grounded in what’s true today.

We assess your sibling’s needs, your capacity, your concerns, and your goals.

This phase brings immediate relief because you finally understand the full picture, without doing the detective work yourself.

You’ll walk away with:

  • Clarity instead of questions
  • Focus instead of overwhelm
  • Priorities instead of pressure
  • Next steps instead of guesswork

Step Two

We uncover your clear path through the system.

This is where confusion turns into forward motion.

Together, we map a realistic path into services, based on what’s actually available, how the system works today, and your family’s real situation.

We’ll clarify:

  • What services are realistically available (and which aren’t)
  • What’s blocking access and how to move past it
  • What needs to happen first, second, and next
  • How living arrangements affect eligibility
  • Which paths aren’t worth pursuing

You’ll walk away with a grounded, real-world strategy that helps you move out of limbo and into action.

Step Three

We help you turn direction into action.

This is where things move from “stuck” to services in motion.

You’re not handed a list and left to figure it out alone. We stay involved while services are being opened, accessed, and activated.

This may include:

  • Guidance through applications, renewals, and service enrollment
  • Coordinating next steps with agencies, case managers, and professionals
  • Preparing you for what each service requires during setup
  • Removing roadblocks when the system says “no” or delays progress
  • Supporting decisions when multiple paths are possible
  • Adjusting the strategy as barriers or timelines change

This is about follow-through inside the system, not managing daily care.

You always know the next step. And you never take it alone.

Frequently Asked Questions

Who is this for?

The Future Is Now™ is built for siblings and aging parents who are responsible for an adult with developmental disabilities and need clear guidance, a realistic plan, and hands-on support without waiting months or years for the disability service system to respond.

Is this worth the investment?

Most families don’t struggle because they lack information. They struggle because they lack clarity, sequencing, and leadership inside a complex system. This program isn’t about Googling answers. It’s about having a guide who helps you understand what’s possible, what isn’t, and how to move forward without wasting time or energy.

What will we walk away with?

You’ll walk away knowing what to do next and why. That includes clear direction, a realistic path into services for your sibling, prioritized next steps, and confidence about how to move forward, without guessing or carrying everything alone.

What if I don’t know what my sibling needs?

That’s exactly where we start. The first phase of the program focuses on clarity so we can understand needs, capacities, and priorities. This ensures decisions feel grounded instead of overwhelming.

What if our situation is complicated?

Many families come to this work feeling rushed, behind, or unsure where to begin. Complexity is common. We focus on making sense of what’s happening and identifying the next right steps based on your specific situation.

Is this covered by Medicaid?

The Future Is Now™ is a private-pay program. Medicaid funds direct care services, but it does not cover the kind of one-on-one guidance families need to make sense of the system, sequence decisions, and move things forward.

This program exists to fill that gap. Offering it as private pay allows families to get clear, individualized support right away… without being limited by waiver rules or long waitlists.

What is the time commitment

Most families work with me over 3–6 months, depending on how quickly the system moves. For siblings, the time commitment is typically under five hours per week.

Do you work outside Kentucky?

Momentum Family Strategies is rooted in Kentucky’s disability and Medicaid systems, where our work is most hands-on. We also support families outside Kentucky on a case-by-case basis. Reach out here, and we’ll talk through what support may be available in your state.

What happens after the program ends?

The Future Is Now™ is designed to create clarity and forward momentum within 3–6 months. Some families choose to continue working together afterward to maintain progress or address new challenges. If that’s a fit, we’ll discuss options when the time comes.

Ready to Build a Future That Works For Both of You?
